Complete this form to grant Made In The Dark LLC ("Producer") permission to record and use your name, image, likeness, voice, and performance in photo / video / audio content.

I ("Participant") grant Made In The Dark ("Producer") and Producer's clients, licensees, successors, and assigns the irrevocable, worldwide, perpetual right to record my name, image, likeness, voice, actions, and performance ("Content") and to use, reproduce, edit, modify, publish, distribute, publicly display, publicly perform, transmit, create derivative works from, and otherwise exploit the Content, in whole or in part, in any media now known or later developed, for any lawful purpose including advertising, promotion, marketing, portfolio/demo reel, websites, social media (organic and paid), broadcast, and internal use.
I understand the Content may be edited, altered, combined with other content, and used with text, graphics, music, voiceover, visual effects, or other materials. I waive any right to inspect or approve any finished product or any copy in which the Content appears.
I acknowledge that good and valuable consideration has been received for signing this Release, the sufficiency of which is acknowledged, even if no money is paid.
I release and forever discharge Producer and Producer’s owners, employees, contractors, clients, licensees, and assigns from any and all claims arising out of or relating to the Content or its authorized use, including claims for invasion of privacy, right of publicity, misappropriation, or defamation, to the fullest extent permitted by law.

This Release is governed by the laws of the Commonwealth of Virginia. Any dispute shall be brought in the state courts located in or nearest to Chesapeake, Virginia, unless otherwise required by law.
